There are approximately 37 million Americans over the age of 64 in the United States. Of them, about 30% suffer from multiple diseases (e.g. diabetes, Alzheimer’s, arthritis, etc.), causing complications to providing care and often needing referrals to multiple specialists. Current healthcare system requires patients themselves to visit various specialists in order to get care. But some patients simply cannot drive or find alternative transportation to visit the doctors, and some others needs to wait for 4-6 months in order to get an appointment. Moreover, the “team” of health providers do not share the same information about their patients, and those who are closest to the patients, family, friends, and home-visit health providers (nurses, care managers and health workers) have no easy way to communicate updates on patient conditions to health providers.
